Disassociation Between Anxiety Levels and Blood Pressure in Pre-Hemodialysis Patients: A Cross-Sectional Study

Abstract


Background: Patients with chronic kidney disease undergoing hemodialysis, both new and long-term patients, tend to experience anxiety due to the need for lifelong hemodialysis. Anxiety levels are often associated with changes in blood pressure, but there is no clear identification of this relationship in patients before hemodialysis. Objective: To determine the relationship between anxiety levels and blood pressure in pre-hemodialysis patients. Methods: This study used a quantitative approach with a cross-sectional method. A total of 152 respondents were taken using total sampling from patients with chronic kidney disease undergoing hemodialysis at the Hemodialysis Unit who participated in this study in December 2024. Hamilton Anxiety Rating Scale (HARS) questionnaire was used to measure anxiety levels. Observation sheet was used which included blood pressure and Mean Arterial Pressure (MAP) of patients before hemodialysis. Spearman rank test was used to determine the relationship between the variables. Results: The characteristics of respondents were dominated by males (57.9%, n=88), aged late elderly (32.9%, n=50), with secondary education (73.7%, n=112), not working (61.8%, n=94), and had undergone hemodialysis therapy for more than 1-5 years (57.9%, n=88). The majority of respondents did not experience anxiety (78.3%, n=119). In addition, the majority of respondents had high blood pressure (57.2%, n=87). No significant relationship between anxiety levels and blood pressure in patients before hemodialysis was found (p=0.202). Conclusion: In general, there is no high level of anxiety in hemodialysis patients who have undergone hemodialysis for 1 to 5 years. However, there is a relatively high blood pressure in hemodialysis patients, which is likely caused by pathophysiological factors of chronic kidney disease. Hemodynamic monitoring remains necessary for nurses to perform on patients, even if they do not have high anxiety. Further research is recommended to identify other factors related to blood pressure in patients with chronic kidney disease
